Spine Osteotomy
Understanding Spine Osteotomy
Spinal deformities can result from various conditions such as Scheuermann’s kyphosis, neuromuscular disorders, congenital abnormalities, degenerative diseases, and ankylosing spondylitis. Severe deformities often lead to symptoms such as stooping, imbalance, chronic pain, fatigue, and difficulty maintaining a horizontal gaze. When these symptoms become disabling and non-surgical treatments fail, an osteotomy spine procedure may be necessary to correct spinal alignment and restore function.
At Texas Spine Group, we specialize in advanced spinal surgery techniques, including spinal reconstruction, to improve posture, mobility, and overall quality of life for patients with severe spinal deformities.
What is Spine Osteotomy?
Spine osteotomy is a surgical procedure that involves cutting and removing a section of spinal bone to correct spinal malalignment. This procedure allows for realignment of the spine, improving posture and relieving nerve compression. It is often performed alongside spinal reconstruction to stabilize the spine and prevent further curvature.
Indications for Spine Osteotomy
Spine osteotomy is typically recommended for:
- Severe, rigid, or fixed spinal deformities
- Progressive spinal curvature that worsens over time
- Numbness, weakness, or intractable pain due to nerve compression
- Spinal instability affecting balance and mobility
- Postural difficulties, including excessive forward bending (stooping)
For mild or flexible deformities, non-surgical methods such as bracing, physical therapy, or positioning may be sufficient. However, when conservative treatments fail, spinal surgery becomes necessary to achieve proper alignment.
Types of Spine Osteotomy
There are three main types of osteotomy spine procedures, each offering varying degrees of correction depending on the severity of the spinal deformity. A spinal fusion with instrumentation is often performed in conjunction with osteotomy to ensure long-term stability.
Smith-Petersen Osteotomy (SPO)
- Used for small corrections of approximately 10-20° per level
- Involves removing a section of bone from the back of the spine
- The posterior ligament and facet joints are also removed to allow flexibility
- No anterior bone graft is required, as motion through the front portion of the spine aids in correction
- Can be performed at multiple levels to restore natural spinal curvature
Pedicle Subtraction Osteotomy (PSO)
- Recommended for larger corrections of approximately 30°, usually at the lumbar spine
- Involves removing the posterior elements, facet joints, and a portion of the vertebral body along with the pedicles
- Offers greater correction compared to SPO by realigning all three spinal columns
- Particularly useful for patients with flat-back syndrome or severe kyphosis
Vertebral Column Resection Osteotomy (VCR)
- Provides the most significant correction for severe spinal deformities
- Involves complete removal of one or multiple vertebral bodies
- Requires spinal reconstruction using bone grafts or metal cages to maintain spinal stability
- Initially performed through both anterior and posterior approaches but can now be done via a posterior-only technique
Expected Outcomes of Osteotomy Spine
Patients undergoing osteotomy spine procedures can experience significant improvements, including:
- Reduced pain and discomfort associated with spinal misalignment
- Improved spinal balance, allowing for an upright posture without excessive bending of the hips or knees
- Enhanced functional mobility and reduced fatigue during daily activities
- Restoration of a horizontal gaze, improving visual alignment
- Improved cosmetic appearance and self-confidence
- Potential benefits for visceral organ function, leading to better respiratory and digestive health
